The second edition of NDTAC’s Transition Toolkit highlights innovative educational practices in transition by the different stages of a student’s involvement with the juvenile justice system: entry, residence, exit, and aftercare. The Toolkit offers practical strategies and resources to aid administrators and service providers in supplying high-quality transition services for youth.
This self-guided learning module, designed especially for new coordinators and administrators, provides a brief introduction to transition. The module describes the goals and key elements of transition, critical related support services, and additional resources that are available to assist providers with effectively supporting youth during transitions to and from facilities.
